The Written Essay (The Rationale, 1500 words)
Introduction: (Approximately 100
words)
You must clearly state the focus of your assignment by including the
following:

Identifying the case study you have chosen
·
The problem stated on the case study
·
Prevalence/statistics relating to this relevant health
condition within the UK to date.
·
Your Smart Goal
·
Your 3 chosen nursing interventions
Main Body of Written Essay:
(Approximately
1200 words)
You must identify and discuss the rationale (reasoning) for each of the 3
nursing interventions.
·
The nursing interventions refer to the actions the nurse
will take to address the problem and meet the SMART goal.
·
The interventions must be evidence based, relevant to the
problem and aim to meet the SMART goal.
The
interventions must address and
demonstrate understanding of:
·
Health Policy and Health Promotion over the lifespan of
the patient
·
Behavioural distress
·
Communication over the
lifespan of the patient.
·
Physical, behavioural,
cognitive and mental health conditions across the lifespan of the patient.
You
are required to demonstrate knowledge of the evidence base that supports each
intervention.
Your
evidence MUST be academically
credible, using evidence from academic and professional sources.
Academically
credible refers to evidence which is current, reliable and accurately
referenced (See library and Personal development department resources PDD)
Conclusion: (Approximately 200
words)
The conclusion MUST contain an evaluation of the
care planned, together with a summary of how this family member can maintain
optimum health for the future.

There should be no new information in the conclusion.
As this is
an academic assignment, you need to demonstrate understanding of the subject
area, therefore avoid the use of tables, lists and bullet points, as they often
require further explanation.

referencing system must be used. You are required to include in-text
citations as well as a full reference list at the end of your assignment.
